POSITION SUMMARY: The Targeted Case Manager’s role is to assist clients in better understanding how
to use resources in the community, become involved with their community, and to utilize skills in order
to better manage stressful life events in appropriate and healthy ways without the use of substances.
The TCM must be self-motivated and maintain excellent documentation regarding each person served.
This position will maintain a caseload of no more than 30 individuals and ensure compliance of all HIPPA
laws.


ESSENTIAL DUTIES AND RESPONSIBILITIES:
The following is a summary of the major essential functions of this position. The position may require
other duties, both major and minor, that are not mentioned below, and specific functions may change
from time to time.
• Provide holistic and comprehensive case management to include:
o Establish goals for treatment and help clients remain motivated for change.
o Engage clients in quality assurance measures quarterly
o Participate in individual and group supervision monthly
o Document all sessions with clients and ensure all information in their chart is current
and up to date
o Meet all billing expectations.
o Refer clients to appropriate health providers and legal entities when necessary
o Monitor client’s emotional wellbeing and behavior
o Connect clients with family and community resources
• Participate in staff and state regulatory training as required
• Adhere to program policies and procedures while maintaining a professional code of ethics
• Maintain a valid driver’s license

EDUCATION/EXPERIENCE/CERTIFICATION:
• Bachelor’s Degree in Social Work or other Human Services degree required.
• One year of post bachelor’s degree experience required.
• Substance abuse experience preferred.
• Appropriate State and Federal Clearances

KNOWLEDGE/SKILLS/ABILITIES:
• Knowledge and understanding of recovery principles and recovery driven service system
• Knowledge of community health resources that support life improvement in physical, emotional, mental and spiritual health, economic security, employment, positive social and
family interdependence and positive cultural identity
• Knowledge of the physical, emotional, intellectual and spiritual impact of alcohol and other
drugs on the individual and family
• Knowledge of Federal, State and Municipal laws and/or regulations that regulate the treatment of
alcoholism and drug addiction
• Ability to maintain confidentiality and handle crisis situations in a calm and supportive manner
• Ability to exhibit professional and courteous behavior, consistent with the ARC mission statement,
when interacting with persons of varying backgrounds and education levels to create a safe and
healthy relationship with clients served

WORKPLACE ENVIRONMENT AND ESSENTIAL PHYSICAL REQUIREMENTS
• Ability to sit and/or stand for long periods of time
• Ability to read detailed health records
• Ability to lift and carry up to 10 lbs.
• Ability to operate computer and other equipment in the office
• Must have sufficient mobility to move around the office and the community
