Education

  •    Master's Degree in Counseling
  •   Master's Degree in Social Work

Qualifications

  • Minimum of 2 years post-Master's degree-direct clinical and other substance abuse service to adult.
  • Communications skills to relate to all levels of management and  staff.
  • Ability to provide both direct service and supervision
  • Team player with interdisciplinary team of varied professionals
  • Knowledge of referral procedures and admittance criteria for health care and community providers
  • Ability to network and collaborate with healthcare and other social agencies who can provide services to clients and families
  • Knowledge of 12-Step Recovery Program
  • Knowledge and competency in problem solving and team building
  • Computer literate with experience in Windows environment

Licensure/Certification

  • Licensure to practice clinical discipline in the appropriate state, as applicable, required in social work and social work supervision.

 

Roles and Responsibilities

  • Coordinates and supervises all case management staff members and activities, which may include monitoring groups, individual therapy, and charting to maintain the highest standard of quality and effectiveness.
  • Conducts reviews of  Case Manager notes.
  • Ensures patient confidentiality
  • Display a professional disposition towards staff and clients
  • Maintains an effective clinical organizational structure and delegate appropriate authority for completion of assigned responsibilities. 
  • Functions as leader of multi-disciplinary Case Management Treatment Team
  • Knowledge of administrative, personnel and clinical policies and procedures, and ability to comply and communicate these clearly and accurately to staff.
  • Adheres to Confidentiality requirements as outlined in 42 CFR Part 2
  • Attention to detail and organization
  • Ability to prioritize work and work related responsibilities
  • Ability to work independently with minimal supervision
